Please read all instructions BEFORE continuing to online registration.
To Begin Online Registration:
Click HERE or the Login button in the upper right hand corner
Click the link that says “I am and new account or student“
Fill in the Account Registration Form
Read and Sign the Parent Policy Acknowledgment Form
Fill in the required Green items on the Create Account Page
If you have more than one student, click the Add Student button and fill in the formClick “Save and Register Students for Classes”
Pick class(es) for your student(s) and click request class on the right (it should say PENDING APPROVAL)
Once finished selecting all classes, scroll to the top and click SUBMIT REGISTRATION
Your registration is now completed.

1. Fill out and sign the 2019-2020 Registration Form
2. Fill out and sign the Emergency Medical form. 2019-2020 Emergency Authorization Form
3. Initial and sign the Policy Acknowledgement form. 2019-2020 Policy Acknowledgement Form
4. Calculate your monthly tuition on the pricing chart.
5. Write a check for the first month’s tuition plus the registration fee.
6. a. Register in person – bring the three forms and check to the academy.
b. Register by mail – mail the three forms and check to the academy address below.
c. Register by e-mail – send an e-mail to firstname.lastname@example.org (click button at right) with the student’s name, class(es) taking, phone
number and credit card information (see below). This will hold a spot in the class but signatures will still be required to
d. Register by phone – call the academy (614-789-0052) with the name of the student, class(es) taking and credit card
information. This will hold a spot in the class but signatures will still be required to complete registration.

Some general notes on registration regardless of the category your dancer fits into:
CDA accepts cash, checks, VISA, Discover and MasterCard.
CDA season registration includes one tuition payment and a registration fee.
Registration is not considered complete until this payment is made.
CDA will enforce a maximum class size in order to provide the best possible teaching environment. Class size is determined by age level, art form, class level and studio size. Register now to ensure your spot in class.
